Upon completion of the form below all contact and order information will be sent to our fundraising department. You will receive a confirmation email within 24 hours of registration.
  • Payment must be received in full in order to hold a foursome.
  • Name of team captain must be submitted (others may be submitted 2 weeks prior to event)
  • Golf is limited to the first 30 foursomes (must pay in full)
  • Golf registration from 7-8am; Shotgun start at 8am
  • Golfers will receive Outing Golf Shirt, Lunch-in Ticket among other items
  • Lunch-in is open to all friends and family (Tickets available any time up to event)
  • Lunch-in will feature buffet, open bar, and prize raffle
  • Lunch-in will begin right after Golf Outing at The Fireside www.FiresideBarandGrille.com
  • Proceeds to benefit the Tackle ALS Foundation, Inc. www.TackleALS.org
  • More detailed information will be emailed to the team captains closer to the event
